Job Summary: As a Homecare Assessment Nurse, you will work autonomously, demonstrating good judgment while adhering to company policies. Your responsibilities include conducting regular visits to clients' homes to provide critical assessments and foster strong first impressions. This role demands meticulous attention to detail, calmness under pressure, and proficient communication skills. Work Hours: Flexible schedule to accommodate varying client appointments. Qualifications: Current certification in standard first aid and CPR. Current LPN or RN license in this state. Valid driver's license and reliable transportation. Must be at least 18 years old. Clean criminal background check. Must possess a State ID. Eligibility to work in the U.S. Current listing on Nurse Registry. Key Responsibilities: Client Interaction: Contact clients to schedule visit appointments. Introduce yourself and provide essential information about AnswerCare. Conduct initial and ongoing assessments, covering personal information, home conditions, and safety features. Gather comprehensive details on the client's medical history, mental and behavioral conditions, routines, and preferences to craft a personalized care plan. Make necessary updates to the care plan as client needs evolve. Maintain open lines of communication with nursing staff, schedulers, supervisors, families, and case managers. Assessments and Monitoring: Conduct assessments every 60 days, accompanied by Satisfaction Surveys. Record vital signs during visits and document any significant changes. Carry out unplanned assessments in emergencies or as requested. Monitor client conditions and supervise caregivers to ensure compliance with the care plan. Advocate for clients' rights while maintaining their confidentiality. Communication: Effectively convey relevant information to enhance care planning. Prepare clinical notes and regularly update the RN at least every 60 days. Coordinate with Case Managers to make necessary adjustments to care plans.
