Key Responsibilities: Conduct comprehensive nutritional assessments and ongoing evaluations of the care plan for each resident. Participate in IDCP care conferences as needed to support resident care. Offer nutrition education and guidance to residents, their families, and staff. Collaborate with interdisciplinary teams to enhance residents' overall health and well-being. Exhibit teamwork skills by working cooperatively with colleagues. Fulfill additional duties as assigned. Ensure compliance with all federal, state, and local regulations and guidelines. Qualifications: Must be a Registered Dietitian or RD eligible. Strong organizational, communication, and interpersonal skills are required. Experience in Long Term Care and familiarity with EMR systems is preferred.
