divh2Pharmacist Caregiver/h2pCoordinates and provides the pharmaceutical care of patients for an assigned clinical unit and participates in patient care rounds with medical staff personnel. A caregiver in this role will provide advanced clinical expertise and leadership in the delivery of anticoagulation services at Cleveland Clinic Hillcrest Hospital. This caregiver will support pharmacists and clinical teams through education, mentorship, and consultation to ensure consistency in anticoagulation management and optimize patient outcomes. The pharmacist will assess clinical metrics, perform quality reviews, and collaborate with interdisciplinary teams to standardize and enhance anticoagulation practices across Hillcrest Hospital. Additionally, this caregiver will represent the Ohio region on enterprise and local committees focused on anticoagulation management, patient safety, and quality improvement./ppA caregiver in this role will work Monday - Friday from 7:30am-4pm./ppA caregiver who excels in this role will:/pulliPerform pharmacotherapeutic monitoring and provision of pharmaceutical care for patients in the area of service responsibility in a routine, continuous manner, taking into account the age and clinical condition of each patient./liliProvide general and patient specific drug information to patients, physicians, nurses and other healthcare professionals./liliInteract routinely with physicians, nurses and other healthcare professionals to provide optimal pharmaceutical care to patients./liliSupport clinical pharmacist and technician educational development by participating in departmental staff development programs and provide leadership in the education of staff involved in providing pharmaceutical care to patients./liliSupport clinical pharmacists in providing pharmaceutical care to patients by direct interaction and formal educational programs./liliSupport and participate in pharmaceutical related research in area of specialty./liliMaintain expertise in areas of practice through continual learning, journal review and other educational activities./liliEducate clinical pharmacists, technicians, pharmacy students, post-graduate trainees, allied health professionals, nurses and physicians in pharmacotherapy within the specialty area./li/ulpMinimum qualifications for the ideal future caregiver include:/pulliPharmD or Bachelors of Science degree in Pharmacy from an accredited college of pharmacy and completion of a post-graduate residency or fellowship training/liliCurrent State licensure as a Registered Pharmacist (RPh) or eligible for reciprocation in current State within 90 days of hire/liliCompletion of a post-graduate year 2 residency (PGY2) or fellowship required OR completion of a post-graduate year 1 residency (PGY-1) plus minimum of 2 years of experience as a clinical specialist or equivalent role, which includes providing pharmaceutical care and performing pharmacotherapeutic monitoring including 50% or more of time spent in the specialty area OR minimum of 4 years of experience as a clinical specialist or equivalent role which includes providing pharmaceutical care and performing pharmacotherapeutic monitoring including 50% or more of time spent in the specialty area/liliFor individuals required to administer immunizations, current Basic Life Support (BLS) certification through American Heart Association (AHA) and completion of a Board of Pharmacy approved course in the administration of immunizations or certification obtained within 90 days of hire/liliFor individuals required to respond to adult or pediatric cardiopulmonary arrest events, current Advanced Cardiovascular Life Support (ACLS) or Pediatric Advanced Life Support (PALS) Certification through American Heart Association (AHA), or certification obtained within one (1) year of hire/li/ulpPreferred qualifications for the ideal future caregiver include:/pulliBoard certification/li/ulpPhysical Requirements:/pulliTypical physical demands include ability to perform the job while standing and walking for prolonged times, approximately 75% of shift and continuously for extended periods of time./liliManual dexterity sufficient to perform computer and dispensing functions of the job; ability to lift up to 30 lbs./liliPosition often includes reaching, stretching, stooping, climbing steps and sitting./liliMust have normal or corrected vision./liliMust have ability to clearly communicate verbally by phone or in person/li/ulpPersonal Protective Equipment:/pulliFollows standard precautions using personal protective equipment as required/li/ulpPay Range: Minimum Annual Salary: $101,900.00 Maximum Annual Salary: $155,390.00/p/div
