We are an equal opportunity employer The Registered Nurse (RN) will provide nursing care and health oversight for individuals with intellectual and developmental disabilities (IDD) living in a state-funded group home. The RN will work collaboratively with the interdisciplinary team to ensure that residents receive high-quality, person-centered care, and are supported in achieving their optimal health outcomes. This position requires clinical expertise, a compassionate approach, and an understanding of the unique health needs of individuals with IDD. Job Duties • Conduct initial and ongoing health assessments for all residents, including monitoring vital signs, physical health, and any chronic conditions or co-occurring health issues. • Develop, implement, and update individualized care plans for each resident, ensuring that health needs are identified and addressed. • Oversee medication administration processes, including the review of medication orders, preparation of medications, and monitoring for any adverse effects or medication errors. • Provide nursing interventions as needed, including wound care, catheter care, G-tube feedings, and other treatments as prescribed by physicians. • Regularly assess the health status of residents, identifying and addressing any changes in condition, and coordinating medical care as needed. • Work closely with residents' primary care physicians and specialists to ensure that medical needs are met, and treatments are appropriately implemented. • Educate residents, families, and staff on health-related topics, including chronic disease management, nutrition, hygiene, and wellness. • Train and supervise direct care staff in health-related areas, including medication administration, infection control, and recognizing signs of illness or distress • Ensure that all nursing activities comply with Maryland state regulations, Medicaid waiver programs, and other applicable healthcare laws and guidelines. • Other duties as assigned. Knowledge & Abilities Possess knowledge and ability to do client intake requiring both assessment of client history concerning both short and long-term needs; Possess knowledge and ability to implement case management principles; Possess effective oral and written communication skills; Possess effective computer skills; Maintain working knowledge of and adherence to all company policies and procedures; Maintain client and company confidentiality; Posses knowledge and ability to do midline or central venous catheter dressing change; Posses knowledge and ability to prepare parenteral nutrition; Posses knowledge and ability to administer parenteral nutrition; Requirements Registered Nurse License from Maryland Board of Nursing First Aid and CPR certified by the American Heart Association Maintain valid driver's license and acceptable driving record Minimum of 1 years' experience working in a group home or relevant experience (Preferred) Prior experience with medically fragile children or similar demographics (preferred) Minimum AA Degree with two years clinical experience. Minimum of two years Broviac TPN experience.
