This contingent position is perfect for a quick learner who thrives in a collaborative, team-oriented environment.If you're passionate about providing exceptional care and growing your career in a supportive environment, we’d love to hear from you!Pay:Visit Rates with Mileage Reimbursement: $63.00 - $100.00 per visitDischarge Without a Visit: $5.00 - $10.00 per non-visit dischargeTransfer Without a Visit: $10.00 per non-visit transferOrientation and In Service Hours: $30.00 per hourNon taxed mileage reimbursement is paid at $0.50 per mile after 30 miles per dayVisit Rates without Mileage Reimbursement: $68.00 - $105.00 per visitJob Description:Provides services in speech pathology or audiology in accordance with the physician’s plan of treatment. Accepts only patients referred by the patient care manager or designee. Demonstrates competency in the skills required by the agency on at least a biannual basis.Assists the physician in assessing the patient’s level of functioning and the development (and revision, as necessary) of the plan of care by applying appropriate tests for speech, hearing and language disorders. Sets appropriate goals for the patient.Records and reports the patient’s reaction to treatment and any changes in the patient’s condition to the physician and/or patient care manager and case manager.Instructs and advises the patient, family and other health team personnel in the phases of speech pathology in which they may assist the patient. The goal is to teach and promote disease prevention for eventual patient & caregiver independence, utilizing agency materials as appropriate.Recommends hearing tests by audiologist when necessary. Checks hearing aides.Requirements:Active Michigan Speech Therapy licenseReliable transportationValid driver's license and auto insurance in your name as a driverHome health care experience preferredDegree of Travel:Home visits and office meetings as indicated. Must participate in necessary in-service programs. Must have reliable transportation and agency-required auto insurance.
