Serious Illness Clinical Support Specialist

Carolina Caring is seeking a Serious Illness Clinical Support Specialist to support our Nurse Practitioners by preparing charts and managing patient data for patients with serious illnesses. In this role, you will collect and enter patient information, perform medication reconciliation, assist with audio-visual visits, document care in the medical record, and help coordinate referrals, authorizations, and hospital notifications. You will also communicate provider recommendations to patients and families, ensuring seamless care and support. The ideal candidate has experience in healthcare support, clinical data entry, or related settings, strong organizational and communication skills, and a compassionate, detail-oriented approach. Schedule: Monday - Friday 8:30am-5pm This full-time position includes an excellent benefits package, paid time off, and mileage reimbursement. RequirementsEducation: Certified Nursing Assistant/Med Tech or CMA preferred. Must maintain CPR certification. Experience: One-year clinical experience is required. Previous experience in primary care, geriatric care, or palliative care preferred.
