RN Clinical Manager-Rehab Unit

Oak Hills Living Center is an EEO Employer - M/F/Disability/Protected Veteran Status Introduction: We are seeking an organized and compassionate RN ClinicalManager to join our team and provide clinical support and guidance to our residents on the short term rehabilitation unit.The Clinical Manager will be responsible for working with residents to identifytheir needs and goals, developing and implementing care plans, and providingongoing support and monitoring to ensure progress. The successful candidatewill have strong communication and problem-solving skills and be able to workcollaboratively with other team members. Responsibilities: Work with residents to identify their needs and goals, including conducting assessments and gathering relevant information Develop and implement care plans to help residents achieve their goals, including coordinating with other service providers as needed Provide ongoing support and monitoring to ensure progress, including conducting follow-up meetings and check-ins Develop and maintain a good rapport with personnel to ensure that services can be maintained to meet the needs of the residents. Supervise unit staff such as RNs, LPNs, CNAs Collaborate with other team members to ensure continuity of care and services Other duties as assigned Qualifications: Must possess a current unencumbered active license to practice as a Registered Nurse (RN) Strong communication and problem-solving skills Ability to multitask and handle a high-volume workload Ability to work collaboratively with other team members Shift Requirements: Full time; 40 hours per week On call rotation every 8th weekend Benefits: • Medical/Dental/Life Insurance • 401K • PTO • Paid Holidays Please visit our careers page to see more job opportunities.
