PermanentPharmacist | Rotating ShiftLocation: Watertown, NY 13601 Shift: Rotating 8-hour Shifts (Days, Evenings, Occasional Nights) Annual Salary: $105,206.40 - $121,971.20 Responsibilities: Participate in interdisciplinary patient care rounds, collaborating with physicians, nurses, and other healthcare professionals to optimize patient outcomes. Perform medication reconciliation by comparing patients' medication history with inpatient medication orders. Evaluate medication regimens for appropriateness in accordance with hospital policies and evidence-based guidelines (e.g., antimicrobial stewardship, renal dosing, therapeutic drug monitoring). Review all medication orders for appropriate dose, duration, frequency, indication, and check for potential drug interactions and contraindications. Communicate and resolve medication discrepancies with prescribers and document interventions and outcomes. Act as a medication expert, offering drug information and support to clinical staff. Compound sterile IV preparations in compliance with USP 797 and USP 800 standards. Ensure compliance with New York State and federal pharmacy laws, Joint Commission medication management standards, and CMS requirements. Provide guidance and mentorship to pharmacy technician staff to promote safe and accurate medication handling and dispensing. Qualifications: Education: Doctor of Pharmacy (PharmD) degree from an accredited pharmacy school Licensure: Active and valid New York State pharmacist license required.
