Position Summary: Eventus WholeHealth is dedicated to delivering comprehensive healthcare services to adults residing in skilled nursing and assisted living facilities. We achieve this through a collaborative network of health professionals including Physicians, Nurse Practitioners, Physician Assistants, Medical Assistants, and in-house Support Staff. Essential Duties and Responsibilities: As a member of our team, you will provide essential primary care services in alignment with Eventus WholeHealth protocols while adhering to the standards of family medicine, geriatric medicine, and internal medicine tailored for long-term care settings. Quality/Compliance: Review and sign MARs upon request, and promptly address all necessary orders related to Primary Care while at the facility or within 24 hours. This includes but is not limited to phone orders, DME orders, Death Certificates, Plans of Care, and hospice orders related to patient care needs. Complete documentation and submit billing records according to Eventus WholeHealth policies. Ensure that facilities receive a hard copy of your electronic documentation within 48 hours post-service for the patient's medical record. Foster open communication with facility staff and administration. Provide relevant feedback regarding medications and patient care to enhance collaborative efforts. Meet with the designated facility care coordinator daily before and after patient care visits. Maintain knowledge of and adherence to Eventus WholeHealth standards, policies, and relevant legal regulations governing healthcare delivery. Follow all ethical standards of practice. Uphold patient confidentiality at all times, employing secure methods for communication, documentation, and electronic records management as per HIPAA regulations. Consult with your clinical supervisor before declining any requests or recommendations regarding resident care from families or facility staff. Qualifications/Education: A Master's degree in Nursing from an accredited institution and a valid medical practice license in your residing state. All other necessary certifications must be current and valid. Completion of an accredited Physician Assistant educational program recognized by the National Commission on Accreditation of Allied Education Programs. Successful completion of the national certification examination from the National Commission on Certification of Physician Assistants. Possess a valid Physician Assistant license in your state of residence. Specialization in geriatric, family, or psychiatry preferred.
