This is an on-site position located at our Lillington, NC branch. Please note that remote, work-from-home, and hybrid arrangements are not available for this role. The Home Health RN Clinical Manager plays a crucial role in coordinating and overseeing all direct patient care services provided by clinical staff. This position is primarily focused on ensuring high-quality home health services and operational efficiency. Key Responsibilities: Develop, implement, and analyze clinical operations tailored to the specific needs of your location. Conduct and delegate patient assessments and reassessments, ensuring compliance with company, physician, and regulatory standards. Oversee caregiver assignments and manage the delivery of care to all patients. Evaluate case referrals and ensure patients meet admission criteria. Guide and instruct clinicians to enhance care performance, addressing their needs and challenges proactively. Assist clinicians in establishing therapeutic goals and developing patient Plans of Care (POC). Monitor compliance with documentation regulations and manage audits and billing processes to meet Medicare standards. Facilitate communication between team members, physicians, and caregivers to ensure cohesive care and effective outcome planning. Work with branch leadership and finance teams to set revenue and budget goals for the location. Engage in sales and marketing initiatives to promote services. Supervise all clinical employees, ensuring adherence to company policies and handling employee relations fairly and objectively. Participate in hiring, training, and evaluating direct care clinicians, recommending continued education as necessary. Assess staff educational needs through consistent documentation reviews and implement training programs accordingly. Utilize Performance Improvement principles to identify and analyze problems and improvement opportunities. Review and ensure the accuracy of clinical staff documentation related to patient care. Enhance Quality Assurance/Performance Improvement (QA/PI) activities to track and analyze care outcomes. Occasionally provide direct patient care during emergencies as needed. Act as Branch Director in absence. Ensure caregiver clinical documentation meets internal and regulatory standards. Qualifications: Associate degree in Nursing (ADN) required, with a Bachelor of Science in Nursing (BSN) strongly preferred. Active and unrestricted Registered Nurse (RN) license in North Carolina. Current CPR and/or BLS certification. Minimum of 2 years of experience as an RN, specifically in home-based care settings such as home health or hospice. Management and leadership experience is preferred. Familiarity with OASIS and Homecare Homebase (HCHB) is preferred. Basic understanding of CMS PDGM is beneficial. Valid driver’s license, auto insurance, and reliable transportation are required. Additional Information: Normal Hours of Operation: M-F / 8 AM - 5 PM (ET) On-Call Expectation: Yes, with a rotating on-call shift. Branch Size: 285+ Census (4.0 STAR rating) Annual Bonus Eligibility: Yes, with quarterly and annual payouts. Salary Range: $77,200 - $106,200 per year, with an incentivized bonus plan based on performance.
