Therapy/Rehabilitation/PT Home Health

Title: Physical Therapist (PT) - Home Health / Hospice Location: Dublin, OH Duration: 13 weeks Shift: Day shift, 5x8 hours (8:00 AM - 5:00 PM) Job Responsibilities Provide physical therapy services to geriatric patients in a home health/hospice setting Manage an average caseload of 5-7 patients per day Complete Start of Care (SOC) visits when required Perform timely, accurate clinical documentation and maintain clear communication Collaborate with interdisciplinary care teams Participate in on-call rotation (approximately every 6th weekend) Travel within an assigned territory up to a 60-mile radius Adhere to professional standards, policies, and dress code requirements Required Skills, Certifications & Qualifications Minimum 1 year of recent Physical Therapy experience Home Health and/or Hospice experience required OASIS experience required for SOC visits (Home Health) Proficiency in HCHB (Homecare Homebase) EMR required BLS certification required Active PT license in hand at time of submission Valid driver's license and reliable personal transportation Strong time management, documentation, and communication skills Ability to work independently in the field Willingness to be flexible with territory coverage NO first-time travelers
