Nurse Practitioner / Physician Assistant - In-Home Primary Care Location: Orlando, Edgewood, Belle Isle, Winter Park, Maitland, Eatonville, Apopka Schedule: Full-time, Monday-Friday, 8:00 AM-5:00 PM EST Visits: ~5-7 per day On-Call: Paid, telephonic only, every 6-8 weeks Assignment Length: 90 days About the Role We're seeking an experienced Nurse Practitioner or Physician Assistant to provide in-home and community-based primary care to patients across the Orlando area. This is a highly impactful role focused on preventing avoidable ER visits and hospitalizations by delivering longitudinal, relationship-based care. You'll work closely with a registered nurse and collaborate with each patient's primary care provider to identify red flags, manage chronic conditions, and keep patients healthy at home. This is not a one-time assessment role-patients are seen regularly based on need, ranging from twice a month to once every three months. What You'll Do In-Home & Community-Based Care Conduct comprehensive in-home assessments, including medical history, physical exams, and medication reviews Provide care in homes, assisted living facilities, skilled nursing facilities, group homes, and other delegated locations Diagnose conditions and develop individualized treatment plans Manage chronic diseases such as diabetes, hypertension, and cardiovascular conditions Prescribe and adjust medications while educating patients on safe use Promote preventive care, including immunizations, screenings, and lifestyle counseling Serve as the patient's primary care provider of record Patient Support & Follow-Up Provide ongoing education to empower patients in self-management Build strong rapport through compassionate bedside manner Conduct regular follow-up visits to evaluate progress and adjust care plans Maintain accurate EMR documentation within 48 hours of each visit Collaboration & Quality Work closely with physicians, RNs, and other care team members Participate in quality improvement initiatives Adhere to all regulatory, compliance, and privacy standards On-Call Coverage Participate in paid telephonic on-call rotation every 6-8 weeks Provide 24/7 support to patients during on-call weeks Travel Travel between patient homes within assigned territory Must have reliable transportation and valid insurance Qualifications Required Active, unrestricted NP or PA license in the state of practice Certification as FNP, AGNP, or PA Active DEA license Current BLS certification Strong clinical assessment and diagnostic skills Proficiency with EMR systems Valid driver's license (3+ years), clean driving record (7+ years), and reliable insured vehicle Ability to pass comprehensive background and drug screening Preferred 2+ years of primary care or longitudinal care experience Home-based visit experience HEDIS/STAR or HCC documentation experience Physical & Work Requirements Ability to lift up to 20 lbs Ability to sit, stand, bend, kneel, and navigate various home environments Frequent driving between patient locations Ability to interpret data, problem-solve, and make clinical decisions independently Comfort working in patient homes, including variable environmental conditions
