In this role, you will be responsible for creating, implementing, and managing individualized plans of care for patients in their homes. The ideal candidate has prior home healthcare experience, strong clinical judgment, excellent communication skills, and a genuine passion for patient-centered care.This position requires travel to patient homes across Atlanta, Cobb, Gwinnett, Henry, and Clayton Counties. The RN will play a vital role in coordinating care, monitoring patient conditions, and ensuring high-quality clinical outcomes in a home-based setting.ResponsibilitiesConduct comprehensive patient assessments in the home settingDevelop, implement, and update individualized plans of careCoordinate care with physicians, caregivers, and interdisciplinary team membersAdminister medications and treatments as prescribedMonitor patients for changes in condition, side effects, or adverse reactionsAccurately document patient visits, care provided, and clinical observationsEducate patients and families on care plans, medications, and health managementCommunicate changes in patient condition promptly to supervisors and physiciansEnsure compliance with state regulations, agency policies, and care standardsQualificationsCurrent Registered Nurse (RN) license in the state of GeorgiaPrevious home healthcare experience REQUIREDStrong knowledge of current nursing practices and home health protocolsExcellent interpersonal, communication, and patient care skillsAbility and willingness to travel to Atlanta, Cobb, Gwinnett, Henry, and Clayton CountiesValid driver’s license and reliable transportationCurrent First Aid and CPR certificationStrong organizational skills and attention to detailFlexible work from home options available.
