Ensure timely and proper supply delivery for patients, assisting them in understanding vendor processes. Monitor clinic and patient inventory, placing orders as needed to avoid disruptions in care. Assist with equipment management, including tracking and retrieval. Support improved patient outcomes through data tracking and analysis. Conduct laboratory tests and communicate critical values promptly. Educate and follow up with patients on their treatment plans. Comply with all federal and state regulations for home dialysis programs. Train patients and caregivers in self-care practices as per standards. Document all nursing activities thoroughly in the Electronic Medical Record to ensure care continuity. Engage with the care team to develop and implement patient care plans. Participate in infection control efforts and adhere to safety protocols. Conduct home visits to assess the environment for optimal patient care.
