Your Role as a Registered Nurse Case Manager Play a vital role in our care team, ensuring each patient lives their best quality of life. Coordinate and manage personalized hospice plans of care tailored to individual needs. Provide hands-on nursing care, emotional support, and guidance to patients and their families. Educate and empower caregivers as they navigate through their hospice journey. Work alongside physicians, social workers, and chaplains to deliver holistic care. React swiftly and compassionately to changes in patient conditions. What to Expect This full-time RN Case Manager position involves 40 hours per week, Monday through Friday, plus a rotating on-call schedule for evenings and weekends. You'll be working independently while having the continuous support of a compassionate care team. Your Qualifications Active Illinois Registered Nurse (RN) license. Graduate of an accredited nursing program (BSN preferred). Minimum of 1 year of experience in medical/surgical, oncology, or hospice nursing (2+ years preferred). CHPN certification preferred, or obtainable within two years. CPR certification required within three months of hire. Strong clinical judgment and the ability to work autonomously. Local residency with the capacity to respond to on-call within 30 minutes. Compensation & Benefits Annual salary range of $78,000 to $108,000 based on your experience and education.
