Licensed Vocational Nurse - Home Health

In this critical role, you will deliver high-quality, personalized nursing care to patients in their own homes, addressing their health needs and fostering independence. Key Responsibilities: Perform comprehensive assessments of patients’ health status and develop tailored care plans in collaboration with the healthcare team. Administer medications, treatments, and wound care, following physician's prescriptions. Monitor patients’ vital signs, reporting any changes or concerns promptly to physicians. Provide education to patients and their families on effective healthcare strategies, disease prevention, and medication management. Collaborate with other healthcare professionals, including RNs and therapists, to ensure holistic patient care. Document all patient care activities accurately and in a timely manner using electronic health records. Offer emotional support and encouragement to patients and their families during their care. Comply with all relevant laws, regulations, and agency policies, including OSHA and CDC guidelines. Qualifications & Requirements: Graduation from an accredited nursing school and a current LVN license issued by the Texas State Board of Nurse Examiners. A minimum of 1 year of clinical nursing experience is preferred, ideally in home health or a related field, with at least 2 hours of general, critical care, or emergency hospital experience. Strong clinical assessment and critical thinking abilities. Proficiency in infusion therapy and wound care is advantageous but not mandatory. Exceptional communication and interpersonal skills. Able to work independently and manage time efficiently. Proficient in utilizing electronic health records and healthcare software. A compassionate, empathetic approach to patient care. Valid driver's license and reliable transportation. Current Basic Life Support (BLS) certification. Benefits: Competitive pay ranging from $40.00 to $50.00 per hour.
