Humana Gold Plus Integrated is in search of Long-Term Services and Support (LTSS) Care Coordinators (Care Coach 1) to assess and evaluate members' needs for optimal wellness. You will guide members and their families towards appropriate resources that foster care and well-being. The LTSS Care Coordinator employs various strategies to manage a member's health challenges. Understanding professional concepts, regulations, and operational standards will help you make informed decisions and set priorities in your role. Key Responsibilities Conduct home visits with Medicaid members and interact in various care settings - expect 75-90% local travel. Ensure members receive services in the least restrictive environments to maintain their well-being through thorough assessments of care needs. Identify and resolve challenges that affect care effectiveness. Plan and implement interventions tailored to care needs. Coordinate and monitor the case management plan to align with the member's personal goals. Continuously evaluate patient care using assessments, data, and ongoing conversations to ensure progress towards desired outcomes. Connect members and their families to suitable resources, leveraging interactions with other payers, providers, and care teams as necessary. Required Qualifications The Care Coordinator (Care Coach 1) must meet one (1) of the following: A Bachelor's degree in social sciences, social work, human services, or a related field. An active, unrestricted Licensed Practical Nurse (LPN) in Illinois, with one (1) year of experience working with elderly individuals. The Care Coordinator (Care Coach 1) must meet ALL of the following: Reside in or near central to southern DuPage County, Illinois, within 10 miles of the county border of adjacent areas. One (1) or more years of experience in healthcare and/or case management. One (1) or more years of experience assisting Medicare and Medicaid recipients, long-term care services, Home and Community-Based Services, and/or managed care organizations. Intermediate to advanced proficiency in Microsoft Word, Excel, and Outlook. Ability to navigate various electronic information applications/software programs, including electronic medical records. Exceptional communication and interpersonal skills to build rapport with members and stakeholders. Ability to travel regionally for face-to-face engagements with members, families, and care teams. Completion of tuberculosis (TB) screening as this role is patient-facing. A valid state driver's license and auto insurance that meets state minimum requirements are required. Preferred Qualifications Active, unrestricted Licensed Professional Counselor (LPC) or Licensed Social Worker (LSW) in Illinois. Bilingual or Multilingual proficiency in English/Spanish, Arabic, Vietnamese, Amharic, Urdu, or others without limitations in speaking, reading, and writing. Additional Information Workstyle: This is a Field position - providing services at non-company locations like member homes and partner facilities. Travel: 75-90% field-based interactions with members and community partners. Occasional meetings in Schaumburg, IL office may be required. Mileage reimbursement is available for work-related travel. Work Schedule: Monday - Friday; 8:00 AM - 5:00 PM Central Standard Time (CST), with flexibility as needed. Language Assessment: Humana associates engaging with members in another language must undergo a language proficiency assessment. Scheduled Weekly Hours 40 Pay Range $53,700 - $72,600 per year.
