Your key responsibilities will include: Conducting patient visits and managing care coordination Consulting with physicians and the care team to provide detailed patient information Collecting accurate OASIS data Maintaining current medication profiles and reporting timely lab values to physicians Participating in weekly case conferences and monthly case manager meetings Qualifications To succeed as a Registered Nurse (RN) with Enhabit, you must meet the following qualifications: Current RN license in the state of employment A minimum of one year of clinical nursing experience preferred Demonstrated knowledge and skills in current nursing practices Possession of a valid state driver’s license and automobile liability insurance Additional Information Enhabit Home Health & Hospice is an equal opportunity employer.
