Pharmacist - Home Infusion Specialist

Company: Allegheny Health Network Job Description: GENERAL OVERVIEW: This role is pivotal in coordinating pharmacy services to ensure safe and effective care for Allegheny Health Network Home Infusion patients. As a pharmacist, you will provide vital clinical services while adhering to state and federal laws, assisting healthcare providers in delivering optimal patient outcomes. ESSENTIAL RESPONSIBILITIES: Admixture and dispensing of medication and parenteral solutions for patients. Dispense pharmaceutical supplies necessary for patients receiving alternate care. Ensure all quality control requirements are met and maintained. Perform accurate and timely documentation within clinical records. Coordinate the delivery of medications and supplies to patients' homes. Provide thorough information to healthcare providers about drugs' safe and appropriate usage. Engage in active interdisciplinary communication for each patient's care plan. Regularly monitor, assess, and document the appropriateness, effectiveness, and safety of drug therapy. Participate in the Performance Improvement program, developing staff competencies as required by USP 797/800. Assist in training and providing guidance to new employees. Develop key indicators for pharmacy quality improvement efforts. Create computer-supported systems to monitor quality improvement activities. Participate in educational programs for other healthcare professionals and community members. Continuously seek professional growth through seminars and CEU opportunities. Conduct patient and caregiver education tailored to individual needs. Engage with physicians concerning patient care communications and marketing. Fulfill marketing responsibilities as assigned. Participate in an on-call rotation as needed. Adhere to the mission, vision, and core values of Allegheny Health Network Home Infusion. Perform other tasks as directed by management. QUALIFICATIONS: Minimum: Bachelor's degree in Pharmacy or Doctor of Pharmacy degree (Pharm D). Licensed or eligible for licensure as a Registered Pharmacist in the State of PA at hire or to obtain licensure within 120 days. Preferred: Completion of a post-graduate residency program in clinical pharmacy services. Experience or knowledge in admixture procedures with two years of hospital experience or one year in home care practice. Supervisory experience. Pay Range: Minimum: $50.88 Maximum: $81.40 Base pay is determined by the qualifications, experience, and expected contributions of the candidate, along with internal peer considerations, market, and business factors.
