Your responsibilities will include: Coordinating and generating hospice referrals from both established and new accounts. Identifying new market opportunities and cultivating strong relationships with hospitals, physicians, and referral sources. Educating healthcare professionals and the community about hospice services, ensuring a seamless transition of care for referred patients. This role involves daily interactions with patients, families, medical professionals, and community partners to guarantee continuity of care, clear communication, and thorough documentation. Collaboration with agency staff is key to delivering efficient and effective care. You will actively engage with referral sources and partners throughout our service area. Qualifications: A genuine passion for serving patients and families with the highest quality of care. Education: Nursing graduates from an accredited school. Licensure: Current Registered Nurse or Respiratory Therapist license required. Must meet all credentialing requirements as demanded by healthcare facilities. Experience: A minimum of 2 years of experience in a clinical care setting or hospice experience preferred. Requirements: Reliable transportation and the ability to perform physical tasks such as sitting, standing, bending, and lifting a minimum of 25 lbs.
