Position Summary In this contractor role, the MD will be responsible for establishing the Tapestryhealth patient relationship for our chronic care management services. We have openings in and around all major and rural cities. Key Responsibilities Responsible for establishing the Tapestryhealth patient relationship for our chronic care management services. Availability 10+ weekdays per month to conduct approximately 3-75 clinical visits onsite and remotely. These are brief, educational consults simply advising patients of the services they are enrolled in. Tapestry Health will reimburse for mileage as well. Document patient encounters accurately and thoroughly within our platform at time of visit. Provide end of day report to program administrators to track progress for completion of visits. Minimum Requirements Qualified MD/DO Active, unrestricted, medical license(s) in Missouri and multiple other states. Excellent clinical judgment and decision-making skills. Must have reliable transportation to travel to assigned buildings. Commitment to providing patient centered care and improving health outcomes. Familiarity with electronic health record (EHR) systems and healthcare technology. Ability to travel within 1-2 hours of assigned region. Compensation Structure 10-15k monthly, mileage reimbursement, malpractice. 1099.
