Key Responsibilities Manage overall care for an assigned patient panel, tailoring care to individual needs and goals Establish trusting relationships with patients and families, providing clinical and emotional support Coordinate interdisciplinary care plans and monitor the effectiveness of care management strategies Required Qualifications Associate degree in nursing Current RN License is required; Compact License preferred Care management experience is required Certified Case Manager preferred Intermediate skills with MS Office Suite, including Outlook and Teams
