Demonstrates clinically sound decision making skills.EEO/AA/Disability/Veteran Responsibilities Resolves drug distribution problems assuring accurate and timely dose delivery to all patients Interprets Clinical and Pharmacokinetic Lab Data in order to formulate medication monitoring plans. Problem Identification & Resolution Provides drug information to nurses, MDs, and other health care professionals Educational Activities Documents medication use variances (MUVs) and Adverse Drug Reactions (ADRs). Regulatory and Safety Quality Compliance Qualifications EDUCATION Graduation from an accredited College of Pharmacy with a B.S. or Pharm.D. degree. Completion of a residency in hospital pharmacy from a program accredited by the American Society of Hospital Pharmacists is desirable but not required. EXPERIENCE Two years experience in an acute care hospital pharmacy is preferred for this position. LICENSURE Licensed to practice pharmacy in Connecticut (or eligible for reciprocation) SPECIAL SKILLS This individual should demonstrate knowledge of pharmacology and therapeutics, and understand pharmacy systems of drug distribution, and formulary management.
