Texas Licensed Case Manager RN

A company is looking for a Case Manager, Registered Nurse (Remote, Texas). Key Responsibilities Facilitate the delivery of healthcare information and benefits to promote member wellness Utilize a collaborative process for assessment, planning, and care coordination to meet health needs Work with members, providers, and community organizations to address comprehensive healthcare and social determinant needs Required Qualifications Active, current, and unrestricted Texas compact RN licensure Minimum 3+ years of clinical practical experience, preferably with Medicare members Minimum 2+ years of case management, discharge planning, or home health care coordination experience 1+ years of experience with MS Office suites Willingness to work Monday through Friday, 8am to 5pm CST
