A company is looking for a Territory Patient Admission Coordinator (Clerical) in Home Health. Key Responsibilities Collaborates with referral sources, sales teams, and clients to gather necessary information for processing quality referrals Documents all referral-related information and actions in the medical record accurately and timely Ensures prompt attention to referrals received through various platforms and follows up with clients and caregivers as needed Required Qualifications, Training, and Education High School diploma or GED Minimum of one year experience in home care, hospice, or personal care services Prior experience with Electronic Medical Records (EMR), preferably HCHB Experience dealing with a variety of payors in healthcare, such as Medicare, Medicaid, and commercial payers, is preferred
