Key Responsibilities: Utilize specialized care management software to conduct monthly calls with Medicare patients with chronic conditions Build rapport with patients to coach them on self-management strategies and improve health outcomes Implement and update the Plan of Care, connecting patients with necessary community resources Required Qualifications: Current, unrestricted California RN license Proficiency with EHRs and web-based applications 3 or more years' experience as a Registered Nurse Self-directed and able to work independently while meeting performance metrics Passion for nursing and improving patient outcomes
