Key Responsibilities Collaborates with referral sources, sales teams, clients, and local agency operations to gather information for processing referrals Obtains and documents verbal orders for patient evaluations as required by state regulations Ensures timely attention to referrals and documents all referral-related information accurately in the medical record Required Qualifications Licensed PTA, LVN, PT, OT, or RN in the practicing state 3-7 years of experience in facility/physician relationships with knowledge of discharge processes preferred Minimum of one year experience in home care, hospice, or personal care services preferred Prior experience using Electronic Medical Records (EMR) preferred Experience dealing with various healthcare payors, including Medicare and Medicaid, preferred
