Arkansas Licensed Care Coordinator

A company is looking for a Care Coordinator to manage patient outreach and health coaching in a remote setting. Key Responsibilities Manage a panel of 400-450 patients and conduct monthly outreach for continuity of care Collaborate with clinical teams and caregivers to address patient needs and support care coordination goals Document all patient interactions in real time while maintaining HIPAA compliance Required Qualifications Must hold a nationally recognized medical assistant certification or Certified Pharmacy Technician (CPhT) Minimum 2 years of clinical experience in a healthcare setting Experience with Electronic Medical Records (EMRs) and strong computer navigation skills HIPAA-compliant home office setup with minimum internet capability of 50 Mbps download / 5 Mbps upload Must be legally authorized to work in the United States
