About the Home Health/Hospice Team Our Home Health and Hospice teams pride themselves on providing top quality care in the home to those individuals requiring additional healthcare services or during their end-of-life journey. Mission for Position Provide skilled nursing care and general health guidance to patients in the home health and hospice setting so every patient and family receives personalized, compassionate care. Duties & Responsibilities Correctly implement the client Plan of Care established by the physician Observe proper infection control and safety procedures Supervise health assistant services Coordinate care team to meet patient and family needs during end of life Position Type This is a casual position that requires one holiday per year and call rotation one evening every other week and approximately every 11th weekend. Qualifications: Qualifications Current MN RN licensure Current valid MN Driver's License and current auto insurance Ideal Candidate Thrives working independently, but knows when to call on their team Has excellent communication skills Can make decisions with limited information Manages patient-family dynamics with ease Is eager to learn and grow Hospice experience preferred but not required Benefits Lakewood Health System is growing and looking for great people to join our mission of providing personalized, quality healthcare for a lifetime.
