A company is looking for a SC Licensed Clinician: LMSW, LCSW or LPC in Behavioral Health. Key Responsibilities Provides customized care and treatment plans for patients within assigned Care Pods Coordinates behavioral health care with primary care providers and monitors patient symptoms and treatment adherence Documents patient progress and collaborates with the care team to establish care management plans Required Qualifications Master's degree in Social Work, Counseling, or a related field from an accredited program Licensed to practice in South Carolina as an associate clinician under supervision or as an independent clinician A secure, high-speed internet connection capable of supporting video conferencing and cloud-based systems
