RN Supportive Care Case Manager

A company is looking for a Supportive Care Case Manager RN - REMOTE. Key Responsibilities Coordinate care for patients with complex needs, ensuring alignment with their treatment goals Facilitate communication between patients, families, and the healthcare team to support care transitions Assist patients in developing advance care plans and navigating available resources Required Qualifications Minimum of 2+ years experience as an RN in Hospice or Palliative medicine Minimum of five (5) years experience as an RN Current RN License required; BSN preferred Current CPR or BLS certification Intermediate computer skills and proficiency in MS Word, Excel & Outlook required
