State Licensed Transitions of Care Manager

A company is looking for a Transitions of Care Advanced Practice Provider Manager. Key Responsibilities Provide clinical care to patients during the 30-day post-discharge period, addressing gaps in care and reducing preventable readmissions Oversee the clinical performance and management of the APP team within the Transitions of Care program Develop and track clinical and operational KPIs, implement strategies for improvement, and assist with recruitment and onboarding of new APPs Required Qualifications 5+ years of clinical practice as an Advanced Practice Provider (NP/PA) 1 year of experience working with post-discharge patients as an Advanced Practice Provider (NP/PA) 2+ years of management or team leadership experience Active RN and APRN license or Physician Assistant license in the home state, along with national APRN certification Able to work a Monday through Friday schedule
