Care Coordinator (CMA or CPhT)

A company is looking for a Care Coordinator to manage patient outreach and health coaching in a remote setting. Key Responsibilities Manage a panel of 400-450 patients, conducting monthly outreach to ensure continuity of care Collaborate with clinical teams to address patient needs and close gaps in care Document all patient interactions in real-time while maintaining HIPAA compliance Required Qualifications Must hold a nationally recognized medical assistant certification or be a Certified Pharmacy Technician Minimum 2 years of clinical experience in a healthcare setting Experience with Electronic Medical Records (EMRs) and strong computer navigation skills HIPAA-compliant home office setup with minimum internet capability of 50 Mbps download / 5 Mbps upload Must be legally authorized to work in the United States
