Compact RN Case Manager

A company is looking for a Remote RN Case Manager. Key Responsibilities Conduct in-depth telephonic assessments to understand patients' medical and psychosocial needs Develop individualized care plans and provide education on chronic disease management Monitor patient progress and ensure continuity of care through proactive outreach and follow-ups Required Qualifications Active Compact Registered Nurse license in good standing 3+ years of RN Case Management experience, preferably in outpatient or ambulatory care High comfort level with technology, including EMRs and telecommunication tools Understanding of accreditation and quality standards (NCQA, HEDIS) Ability to work independently and manage multiple priorities remotely
