Texas Licensed Advanced Practice Provider

A company is looking for an Advanced Practice Provider NP/PA - Transitions of Care. Key Responsibilities Perform initial TOC visits including comprehensive post-discharge assessments and medication reconciliation Conduct acute symptom TOC/virtual urgent care visits to diagnose and manage patient needs Participate in interdisciplinary TOC rounds to discuss high-risk patients and enhance team collaboration Required Qualifications At least 3 years of experience in a hospital, post-hospital, or primary care setting Ability to work full-time, including one weekend shift per month Active RN and APRN or Physician Assistant license in good standing Active BLS certification at time of hire Willingness to obtain licensure in required states within 90 days of hire
