A company is looking for a Patient Resource Specialist (Remote). Key Responsibilities Act as a liaison between patients and community resources to ensure timely access to essential services Collaborate with care managers and healthcare providers to address patients' social and financial needs Assist patients in applying for Medicaid and connecting them with relevant social services Required Qualifications High School Diploma or equivalent required; college degree in Social Work or related field preferred At least 2 years of experience in case management, social work, or customer service Strong understanding of social determinants of health and their impact on patient outcomes Experience with public assistance programs, including Medicaid, is a plus Proficient with technology, including Google tools and various digital platforms
