Maryland Licensed Care Manager

A company is looking for a Care Manager (Remote). Key Responsibilities Identify members with acute/complex medical and/or behavioral health conditions and develop comprehensive care plans Collaborate with members and the interdisciplinary care team to monitor and update care plans, ensuring alignment with medical benefits Document member data according to application protocols and regulatory standards Required Qualifications High School Diploma or GED required; Bachelor's degree in nursing preferred CCM/ACM or other RN Board Certified certification in case management preferred 5 years of clinically related experience in Care Management or direct patient care RN - Registered Nurse - State Licensure required upon hire Must be eligible to work in the U.S. without sponsorship
