A company is looking for a Telephonic Care Navigator.
Key Responsibilities Completes telephonic outreach to members and assists with scheduling provider appointments Educates members on gaps in care and links them to community resources Maintains confidentiality of health information and ensures adherence to program guidelines Required Qualifications High School Diploma / GED or equivalent work experience 1+ years of call center and/or telephonic customer service experience 1+ years of professional experience in an office setting using telephone and computer 1+ years of experience in healthcare/insurance or social work/community outreach Ability to work full-time, Monday - Friday, with flexibility for shift schedules
