Palliative Care RN

A company is looking for a Supportive Care Case Manager/Palliative Care RN - REMOTE. Key Responsibilities Lead complex Goals of Care discussions and coordinate care plans for high-acuity patients Educate patients on palliative care options and facilitate transitions to appropriate services Assess symptom burden and implement nursing interventions to improve patients' quality of life Required Qualifications Minimum of 2+ years experience as an RN in Palliative medicine required Minimum of five (5) years experience as an RN required 2+ years of care plan creation and proactive discharge planning experience Current RN License is required, BSN preferred Current CPR or BLS certification is required
