Mississippi Licensed Care Manager

A company is looking for a Mom & Baby Care Manager. Key Responsibilities Engage members and their support systems through strength-based assessments and a trauma-informed care approach Develop and update individualized care plans in collaboration with the interdisciplinary care team (ICT) Coordinate communication and collaboration among members, ICT, and community resources to achieve care plan goals Required Qualifications, Training, and Education Nursing degree from an accredited program or Bachelor's degree in a healthcare field, or equivalent work experience A minimum of three years of nursing experience in relevant areas such as case management or care coordination Preferred experience includes three years in Medicaid and/or Medicare managed care and maternity care Current unrestricted clinical license in the state of practice as a Registered Nurse is required Case Management Certification is highly preferred
