Your Role As an RN Case Manager, you are the clinical lead for your assigned patients - supported by a strong interdisciplinary team. You will: • Conduct comprehensive nursing assessments (with Admissions Nurse support) • Develop and manage individualized plans of care • Collaborate with physicians, LPNs, social workers, chaplains & aides • Educate families on symptom management and disease progression • Participate in IDT meetings • Maintain timely, compliant documentation Built to Prevent Burnout We understand hospice RN concerns.
