For this New York United States-based position Expected base pay is $49-$53 (independent license), $47-$49 (provisional license) An employee in this class serves as a liaison with health service providers, persons with substance dependency problems as well as their families when applicable. The employee is expected to exercise judgment and initiative in carrying out the clinical program and treatment goals in the area to which assigned after an intensive orientation. JOB DUTIES A. Provide individual, group or family counseling to maintenance clients B. Maintain therapeutic contact with MAT client caseload during their stay in the treatment program. C. Maintain timely and appropriate client charts. This will include Intake assessment, progress notes, and discharge summaries. D. Maintain close communications with Addiction Specialist MD, and other program staff. E. Supervised by Behavioral Health Director F. Adhere to all State, Federal and Program regulations and procedures regarding methadone treatment. G. Refer clients on an as-needed basis to other community services. H. Develop contracts for problematic clients inclusive of detox and maintenance programs. I. Prepare and present case conferences for caseload maintenance. J. Maintain communication between methadone program and outside agencies when appropriate. K. Communicate clearly and cooperatively with staff and clients. L. Work within program's quality assurance guidelines. M. Participate in professional development training throughout the year; participate in safety training N. Commit to the provision of culturally competent services. O. Follow established company policies and those outlined in the program policies and employee handbook. P. Educate patients on MAT options, expectations, risks, and benefits. Q. Support engagement and retention in treatment. R. Coordinate MAT services (buprenorphine, methadone, naltrexone) in collaboration with medical and behavioral health providers. S. Serve as the primary point of contact for MAT referrals, intake coordination, and treatment transitions. T. Ensure continuity of care during intake, discharge, transfers, or reentry U. Assist with audits and reporting requirements. Key Qualifications • Must have a bachelor's degree, master's degree preferred • CASAC license preferred. • CPR certification • Prior clinical experience working with substance abuse or related fields preferred; have familiarity with relevant research literature, clinical assessments, procedures and methods, particularly those designed for substance abuse; have ability to assess treatment, goals and write goal-directed, individualized treatment plans; have ability to monitor and document individual behavior patterns and modify treatment plans; and be familiar with diagnostic nomenclature outlined in the most recent Diagnostic and Statistical Manual. • Correctional experience preferred. • Minimum 2 years' experience in substance use treatment, behavioral health, or care coordination. • Knowledge of MAT principles and evidence-based practices • Strong organizational, documentation, and communication skills YesCare Benefits (For Full-Time Positions) Health, Dental, and Vision Insurance Life and Disability Insurance Generous PTO Plan 401k with Matching Funds Employee Discount and Rewards Program Tuition Assistance Continuing Education Reimbursement Why Join YesCare?
