Details: Setting: hybrid remote and in-patients' homes role Location: Bloomsburg, PA 17815 Schedule: Mon-Fri (8a-5p includes travel time for in-home visits) Duties: APP's serve multiple serious conditions like CKD and/or ESRD/ESKD Conducts assessments including annual wellness exams Counsels and educates patients and their families about programs Responsible for coordination of care with primary care providers Part of the Integrated care team working with RN and Social Worker Prescribe medications, order tests, collaborate with Monogram physician Follow up and evaluate patient's status Required : NP, FNP, AGNP, ACAGNP with Active PA license Board Certified- ANCC or AANP 2 or more years of direct patient care DEA, BLS Compensation : Discussed by phone Receive a paycheck every Friday Mileage reimburseed 100% of malpractice insurance covered If interested please email CV to: or
