Job Summary: As a Case Manager, you will be the guiding light for patients and families throughout their care journey. Your role will involve coordinating services, connecting resources, and crafting personalized care plans that prioritize the unique needs of each patient. You will work in close partnership with physicians, nurses, and department leaders to maintain clear communication and ensure safe transitions of care. Key Responsibilities: Assist patients and families from program orientation to discharge by explaining rehabilitation philosophies, Medicare and insurance benefits, and patient rights. Coordinate education efforts for patients, families, and caregivers to encourage active participation in treatment planning, goal discussions, and family conferences. Develop and implement individualized treatment plans that reflect each patient's strengths, needs, and personal recovery goals. Conduct psychosocial assessments and discharge planning evaluations, thoroughly documenting and communicating findings to the care team. Collaborate effectively with the Rehab Program Director, Facility Case Management Director, and interdisciplinary teams to ensure seamless, patient-centered care.
