Fee for Service Rates: Initial & ROC Weekday visit rate: $100 Initial & ROC Weekend/Holiday visit rate: $120 Revisit daily rate: $85 Orientation daily rate: $250 In-Service hourly rate: $55 Weekday and weekend shifts available. As a Community Health Nurse, you will play a crucial role in assisting patients on their recovery journey from the comfort of their homes. Your expertise will enable you to provide comprehensive clinical services, patient assessments, medication management, and education for patients and their families. In this vital role, you will coordinate, implement, and evaluate the home care needs of your patients. This includes reviewing patient insurance information, facilitating individualized care plans, and ensuring effective communication with families and other caregivers. Strong documentation, time management, and communication skills will be essential for your success in this position, where you will enjoy autonomy and respect for your professional opinions. Create and update comprehensive, patient-centered home health plans in collaboration with the physician. Case manage the care services for assigned patients to achieve positive clinical outcomes and reduce rehospitalization risks. Support the financial goals set by the agency. Demonstrate expert knowledge in community health nursing practice. Current NYS RN license and registration are required. Bachelor's degree in nursing is preferred; an Associate's degree is required. A minimum of one year of acute medical-surgical nursing experience is required. Experience with Certified Home Health Agencies (CHHA) is preferred.
