In this role, you will focus on managing patient referrals, forging partnerships with local hospitals to secure those referrals, and educating the community and referral sources about the importance of hospice care. You will play a crucial role in ensuring seamless communication, continuity of care, and accurate documentation in collaboration with patients and their healthcare teams. Build and maintain relationships with healthcare providers and referral sources. Educate the community and medical professionals on hospice services. Interact daily with patients, medical teams, and referral sources to ensure comprehensive care. Collaborate with agency staff to enhance patient care effectively and efficiently. Engage in tasks outside of the office to build connections within the local community. Qualifications: A genuine desire to serve patients and families, along with a passion for quality care. Education: Minimum of an Associate Degree in any health-related field or equivalent work experience. Experience: A minimum of 2 years of sales experience in a clinical care setting; hospice experience is preferred. Requirements: Must have reliable transportation and the ability to stand, sit, bend, and lift up to 25 lbs.
