The Clinical Team Leader will play a vital role in overseeing and mentoring primary care providers (PCPs) at our assigned center. You will be accountable for enhancing clinical quality and patient satisfaction while also delivering a portion of direct patient care as a PCP, depending on your team size. Your leadership skills will drive clinician engagement and promote a strong culture of clinical and operational synergy. Key Responsibilities: Deliver high-quality care for patients with both acute and chronic conditions, particularly in an older adult population. Take full responsibility for patient care outcomes, coordinating with specialists as necessary while maintaining oversight of detailed care plans. Engage with hospitalists regarding patient admissions to ensure optimal continuity of care. Conduct thorough assessments, diagnoses, treatments, and care coordination to address the needs of patients with complex health challenges. Lead a care team that includes medical assistants, facilitators, and coordinators, ensuring holistic patient management. Work closely with transitional care teams for patients who are unable to visit the office, connecting with case managers and other health professionals as needed. Facilitate weekly Super Huddles and Transforming Care Meetings, providing guidance and support to center leadership. Actively participate in center operations to maintain quality services, engage in patient recruitment, and assist in the hiring process for new providers. Mentor and coach PCPs to foster outstanding clinical outcomes within the team. Collaborate with operations leaders to enhance the financial performance of the center and increase membership numbers. Perform other duties as necessary based on management direction. Required Skills and Qualifications: Competency in Microsoft Office Suite and other relevant applications. Willingness to travel locally as needed for training and recruitment initiatives. Fluency in English is essential, with additional language skills beneficial. The ability to exercise independent judgment is crucial for this role. Education and Experience Requirements: MD or DO in Internal Medicine, Family Medicine, Geriatrics, or a similar specialty. Current and active MD license in the state of employment. At least 1 year of clinical experience in geriatric, adult, or family practice settings is preferred. Completion of ChenMed training programs is expected. Board certification in a relevant specialty is preferred; board eligibility is required. A valid DEA number for controlled substances is necessary. Basic Life Support (BLS) certification is required within the first 90 days of employment.
