The Home Dialysis Registered Nurse is a vital role focused on empowering patients who choose home dialysis options like hemodialysis or peritoneal dialysis. This position is dedicated to providing exceptional, quality care, ensuring patient safety, and delivering outstanding customer service in all interactions. Qualified candidates with prior experience in peritoneal or hemodialysis may be eligible for a sign-on bonus of up to $5,000. Responsibilities include: Training patients in compromised settings and home visits to support patient learning and care. Training Provided: Candidates with backgrounds in in-center hemodialysis, critical care, or home health will receive comprehensive training. Work Schedule: Monday to Friday from 8 AM to 5 PM, with flexible scheduling based on patient needs. On-Call Pay: Enjoy competitive compensation when placed on-call, with rare holiday work required. Essential Duties Include: Adhering to all federal, state, and local regulations related to home dialysis practices. Training patients and care partners in self-care practices for peritoneal and hemodialysis. Conducting home assessments and routine evaluations to ensure optimal care delivery. Documenting all patient interactions and care in the Electronic Medical Record to ensure continuity of care. Participating in infection control measures and reviewing emergency procedures with patients. Maintaining an appropriate clinic and patient supply inventory, placing orders timely. Engaging in continuous quality improvement efforts through data tracking and trend analysis. Administering medications and conducting routine laboratory tests as needed. Being available for on-call nursing duties during nights and weekends. Exhibiting flexibility to meet patient and program demands. Additional Responsibilities for Home Hemodialysis: Coordinating home evaluations to confirm that all equipment requirements are met. Ensuring water sampling protocols are followed and reporting results appropriately. Qualifications: Must possess a current RN license in good standing. At least 12 months of clinical experience in nursing, with a minimum of 3 months in dialysis. Prior experience with maintenance dialysis care is preferred. CPR certification required and must be maintained. Ability to distinguish primary colors and complete the Home Therapy Nurse Orientation program. Demonstrated verbal and written communication skills in English. Basic computer skills are necessary; proficiency in clinical applications required within 90 days of hire.
