Key responsibilities include: Coordinating and conducting patient visits Consulting with physicians regarding patient health Accurate collection of OASIS data Maintaining up-to-date medication records Prompt reporting of lab results to physicians Participating in weekly case conferences and monthly case manager meetings Qualifications To be eligible for this position, Registered Nurses must fulfill the following qualifications: Current RN license in the respective state Preferred minimum of one year clinical experience Demonstrated knowledge and proficiency in current nursing practices Valid driver's license along with automobile liability insurance Additional Information Enhabit Home Health & Hospice values diversity and is an equal opportunity employer committed to creating a respectful and inclusive workplace.
