Title: Clinical Nurse Coordinator - Telephonic Care Locations: CO-DENVER, 700 BROADWAY FL-LAKE MARY, 3200 LAKE EMMA RD, STE 1000 IN-INDIANAPOLIS, 220 VIRGINIA AVE MO-ST. LOUIS, 100 S 4TH ST FL-TAMPA, 5411 SKY CENTER DR Hybrid: This position requires associates to work in the office 1 - 2 days per week, fostering collaboration and connectivity while also providing flexibility for work-life balance. This hybrid model supports team engagement and individual productivity. Alternate locations may be considered for candidates within a reasonable commuting distance from an office. Please note that candidates outside of a reasonable commuting distance from the specified locations will not be considered for this role unless an accommodation is required by law. Schedule: Monday - Friday, 8-hour shifts. Flexibility may be required for weekends and holidays based on business needs. As a Telephonic Clinical Nurse Coordinator, you will collaborate with healthcare providers and pharmacists to drive personalized health management and enhance health outcomes for our consumers. Primary duties may include, but are not limited to: Conducting care management activities for members with complex and chronic care needs, particularly within oncology and REMS programs. Ensuring the delivery of medically appropriate, high-quality, and cost-effective care. Partnering with physicians and pharmacists for the assessment of care appropriateness, intervention planning, and clinical guidance. Collaborating with providers to evaluate consumer needs, aiming for proactive planning to prevent adverse events and ensure continuity of care. Performing clinical assessments to develop individualized goals and implementing care plans accordingly. Monitoring and evaluating the effectiveness of care management plans and making necessary adjustments. Minimum Requirements: An ASN or ADN with a minimum of 3 years of clinical experience, or an equivalent combination of education and experience. A current, valid, and active unrestricted RN license in the applicable state(s) is required. Multi-state licensure is necessary if providing services in multiple states. Preferred Skills, Capabilities, and Experiences: Experience in oncology care. Prior call center experience. Familiarity with Enterprise Rx. Proficiency in Microsoft Office Suite. Experience with Genesys systems. Contract experience with Pfizer and BMS REMS. For candidates working in person or virtually in the locations listed, the salary range for this position is $34.66 to $51.99 per hour.
